HOW CAN THE FORM WEIGHT LOSS CLINIC HELP ME?

The FORM clinic team is excited to guide you through this journey of weight loss and restoration of your optimal health. In this journey, we will utilize several modalities and members of the care team to ensure you have the support and information you need to be successful. The ultimate goal is not only weight loss that will subsequently improve conditions related to being overweight, but also to improve your overall health by increasing energy, mood, mindset, and longevity. We want you to get back to being and feeling like YOU! We are here to assist you in choosing the best path (or paths) to meet your optimal health goals.


BEHAVIOR MODIFICATION

Behavior modification is the application of therapeutic lifestyle interventions including nutrition counseling, increased physical activity, and stress management skills. These interventions will be included in all aspects of this journey. We will work as a team to find a specific plan for you according to your needs. Behavior modification alone can improve chronic conditions such as cardiovascular disease and type 2 diabetes in addition to improving your overall health and wellness.


CONSULTATIONS

In this journey you will be referred to other members of our multidisciplinary team to assist in providing the best plan to success. These team members may include obesity medicine provider, registered dietitian, counseling services, and physical therapy. Our registered dietitians will work with you to find the best nutrition plan for you. You will learn how to read food labels, identify foods with essential macro/micro nutrients, and how to track your progress. Counseling services help you identify/find the root of unhealthy eating behaviors, assist with stress management, and screen for conditions such as depression and anxiety that may provide a barrier to achieving your goals. In the course of your journey you may be referred to physical therapy for evaluation of physical conditions and education on use of equipment and appropriate home exercises.


WEIGHT LOSS MEDICATIONS

Weight loss medications carry multiple side effects and contraindications to some conditions, thus the assessment of safe use will vary per patient. Weight loss medications may be an option for you. Weight loss medications are not for everyone and the decision to move forward will be evaluated by your FORM team provider. Medications can be a valuable tool in assisting weight loss and improving conditions such as diabetes and heart disease.


BARIATRIC SURGERY

Weight loss surgery has many advantages that will benefit overall health and quality of life. Bariatric surgery, along with improved diet and exercise, can work together to provide you optimum health. This can not only improve the way you feel, but reduce obesity related conditions such as diabetes, high blood pressure, high cholesterol, and sleep apnea. Bariatric surgery requires patients to adhere to pre and post surgery behavior modifications to enhance weight loss outcomes. If this is an option for you, you will be referred to our General Surgeon to discuss the best direction and plan.
