MEDICAL HOME VISITS FOR PEOPLE MANAGING SERIOUS ILLNESS
Home Visits improve quality of life for the patients whose needs are most complex. The program offers specialized medical care focused on relief of symptoms and stress.
Working in partnership with your primary care provider and a team of medical professionals, home visits focus on:
- Symptoms relief for patients
- Care alongside curative treatment
- A team approach for patient care
- Stress relief and an extra layer of support for patients and their family
Patient Qualifications
- Discharged from an acute care hospital, and unable to visit their primary care provider within day 1-3 of discharge.
- High Risk for readmission to the hospital or emergency department.
- Chronic illness
- Need a nurse practitioner for symptoms and comfort checks
- Face-to-Face assessment of the patient unable to come to the physician office.
- Home Health patient needing an evaluation and management in their home.
